The Serie A side trailed 4-1 to Barcelona from the first leg at the Nou Camp, but thrashed the LaLiga leaders 3-0 on their home patch.
Edin Dzeko gave Roma the lead with a composed finish, before Daniele De Rossi doubled their lead from the penalty spot.
Manolas then headed in the all important third goal, as the Italian club progressed on away goals rule.
In the other game, Liverpool beat Manchester City 2-1 at The Etihad.
Gabriel Jesus gave the hosts a 2nd minute lead, but goals from Mohammed Salah and Roberto Firmino, ensured that Jurgen Klopp’s men qualified on a 5-1 aggregate.
